We do bulk-bill Medicare EPC plans for Health Care Card holders, aged care pensioners and under 16's.  A $10 discount is offered for Health Care Card holders for private visits.

Additional Information

We are a small, fee-for-service clinic, as a result we collect payment at the time of service (cash, credit card, HICAPS are available). For Workcover, TAC and veteran affairs we will send an invoice direct to either employers or insurance company.  For Medicare EPC we will bulk bill Medicare directly.  We do not charge a gap for Medicare EPC treatments for Health Care Card holders or Pensioners.

Terms and Conditions

Here at Fountain Gate Physiotherapy we are committed to assisting in your recovery. We will:

  • Provide you with a clear treatment plan
  • Book the appointments ahead of time to ensure you are able to secure a time convenient for you
  • Refer on for scans or to the appropriate health professional should we feel that you require further investigations

For the best outcome for you and others we ask that you adhere to the following conditions.

1. Cancellations

Cancellations at the last minute or clients not attending their appointment has a major impact on our waiting list and can alter the progress of your treatment.

SMS reminders are sent the day before your appointment (if you have consented to this) and you are able to confirm your appointment to this message. Our answering machine is operating 24 hours if you need to cancel your appointment outside of business hours.

If:

  • You cancel your appointment with less than four hour’s notice; or
  • You fail to attend your allocated appointment time.

our cancellation policy may apply.  Failing to attend or notify within the four hours may incur a fee of $75.  Initial bookings made online will incur a full fee penalty.

This cancellation fee is applicable to all patients and is NOT claimable on your health fund.

Compensable clients who are claiming their consultation fee through Medicare or WorkCover will be required to pay their cancellation fee themselves. The cancellation fee cannot be bulk billed.

Further appointments will not be booked until the outstanding cancellation fee has been paid.

If we receive notice outside the four hour requirement that you are unable to attend we will be able to offer the appointment to other clients from our waiting list or those requiring urgent physiotherapy.

We greatly appreciate your support and understanding of our policy and value your questions should you have any.

We would like to avoid having to apply a cancellation fee – your assistance with notifying us if you are not able to attend will be greatly appreciated.

2. Late and Missed Appointments

  • If you arrive late to an appointment, but within your allocated time slot, the treatment time you receive may be shortened so as to not inconvenience other clients.
  • If you either arrive after or entirely miss an appointment, you may be charged the consultation fee in full.
  • If you miss your appointment and call to reschedule, you may be required to pay for the missed appointment prior to booking any further appointment dates.

3. Consent to Treatment as per online Client Regstration Form

You hereby give consent to the therapists of Fountain Gate Physiotherapy to perform all necessary examinations, mobilisations, therapy (including taping, dry needling, soft tissue massage), rehabilitation and medical diagnostic procedures that is consistent with their professional training and understanding of your injury.

You understand that you will have an opportunity to discuss with your therapist the purpose of your physiotherapy care before any treatment is performed.

You acknowledge and accept the following:

  • Physiotherapy treatment is physical in nature and may require you to remove some outer clothing
  • You have the right to see the physiotherapist of your choice, refuse intervention or seek a further opinion and to provide feedback and make a complaint.
  • The adverse risks associated with treatment, including stiffness and soreness, soft tissue injury, neurological complications, cerebrovascular injuries, skin irritations from taping methods, burns and other minor complications.
  • The probability of risk is lowered by screening procedures performed during the assessment and reassessment.
  • Other treatment options exist if the risk of physiotherapy is considered to be high, including medication, medical care, hospitalisation and surgery.
  • That should you have a referral or scan results please provide this to your physiotherapist to ensure there is continuity of care.
  • That at times it will be necessary to release information regarding your presenting condition with other treating practitioners such as your GP or surgeon.

4. Accounts and Billing

  • It is expected that all fees are paid in full at the time of consultation.
  • We accept cash, EFTPOS, credit cards and offer electronic health rebates (via HICAPS) for your convenience. However, in the event of electronic network failure, full payment is still required on the day of consultation.
  • Payment of your accounts can be done via the telephone (credit cards only) or in person at the practice.
  • Those who feel they cannot comply with our very strict billing methods, or wish to be given special consideration, please feel welcome to put your request in writing and address it to the Practice Director.

5. Privacy Policy Summary Statement

  • This practice is committed to protecting your privacy and your personal information is generally only disclosed to other members of your medical team where necessary
  • The personal and health information you provide during your consultation and subsequent treatment will be collected for the primary purpose of providing care to you.
  • Where required by law or if necessary for debt collection reasons, your personal information will be disclosed.
  • Inspired Physiotherapy uses SMS and/or email communication to remind you of your upcoming appointment
  • From time to time we may send you information regarding happenings at Inspired Physiotherapy via a number of possible ways including email or SMS.
  • Reasons we may contact you include:
    • To administer accounts and process payments.
    • To communicate with you regarding any issues affecting your treatment.
    • Provide information on services and benefits available to Clients.
    • To notify Clients of promotions and events.
    • Appointment Reminders.
    • Appointment Follow Up calls
    • Marketing and promotional material about new products, services or special offers.
    • Periodic Newsletters.
    • Practice Updates.
    • Market research or surveys to improve our products and services.
  • Should you wish not to receive such communications or our convenient SMS appointment reminders please advise the administration staff or alternatively follow the ‘unsubscribe’ instructions provided in the communication / on the Client Profile.
  • Your acceptance of these Terms and Conditions is considered consent to receive such communications as outlined above however you may opt out at any time.

6. Exclusion of Liability

The Practice Director and employees of this Practice shall not be liable for, nor shall they accept any responsibility for any injury, loss or damage sustained by any person or persons arising out of any of the treatments or procedures delivered in this Practice which does not arise from any negligent act or omission of the Practice Director and employees.

Strengthening and Rehabilitation Group Session Passes Terms and Conditions

1. All group sessions must be pre-booked and paid for in advance. Sessions are filled on a first come first serve basis. The group session initially booked will be automatically booked for the following week unless otherwise rescheduled immediately following the group session.

2. 4 session pass is valid for a period of 6 weeks.

3. 10 session pass is valid for a period of 12 weeks.

4. It is the clients responsibility to use the 4 or 10 session pass within the validity period. Extensions will not be offered unless due to serious injury or illness.

5. If you cancel a group session at least 24 hours prior a catch up class will be offered if available. The offered session cannot be cancelled and is counted as an attendance.

6. Please wear clean socks on the reformers. No shoes are allowed. Please bring a sweat towel for use on the reformer.

7. Fountain Gate Physiotherapy reserves the right to cancel a group session on any given day if necessary. Clients with multi-session passes will be offered a catch up session at their convenience.
